Chronic Hives Treatment

When hives keep recurring for more than six weeks, treatment shifts from short-term symptom relief to long-term suppression of inflammation and prevention of flare activity. Chronic hives, also called chronic spontaneous urticaria in many cases, often require stepwise treatment and close follow-up.

The first step in chronic hives treatment is usually a daily second-generation H1 antihistamine. These medications help control itching and wheals and are often taken every day rather than only when symptoms appear. If symptoms are not controlled at standard dosing, many patients require dose escalation under physician guidance.

For patients whose chronic hives remain active despite standard antihistamine treatment, additional oral medications may sometimes be added as part of a broader treatment plan. Depending on the clinical picture, this can include short-term corticosteroids for rescue treatment or selected adjunctive medications to improve symptom control.

For chronic hives that remain active despite antihistamines, omalizumab is a key targeted treatment option. Omalizumab works by binding IgE, which lowers free IgE levels and reduces downstream mast-cell and basophil activation that contributes to chronic hives. FDA labeling includes chronic spontaneous urticaria in adults and adolescents age 12 and older who remain symptomatic despite H1 antihistamine treatment, with dosing every 4 weeks.

Dupilumab is another targeted option for chronic spontaneous urticaria in patients who remain symptomatic despite H1 antihistamine treatment. Dupilumab blocks signaling from IL-4 and IL-13, two immune pathways that contribute to type 2 inflammation. FDA labeling includes chronic spontaneous urticaria in adults and in pediatric patients age 12 and older who remain symptomatic despite antihistamine therapy
