Hello, my name is Dr. Thomas Knackstedt, and I’m a board-certified cosmetic dermatologist and dermatologic surgeon at Peak Skin Center. In this video, I want to talk a little bit about xanthelasma.
Xanthelasma are soft, yellowish plaques that most commonly form on the eyelids, especially near the inner corners. They are made up of cholesterol-containing deposits in the skin. Xanthelasma are benign, but many people seek treatment because they are cosmetically bothersome or gradually enlarge over time. In some cases, xanthelasma can be associated with elevated cholesterol or other lipid abnormalities, so a medical evaluation of cholesterol levels is often appropriate, especially when xanthelasma appears at a younger age or develops rapidly.
Treatment depends on the size, depth, and exact location on the eyelid. Several options can be effective. Chemical peeling can be used in selected cases to help break down superficial lesions, but it requires careful technique because eyelid skin is thin and sensitive. Cryosurgery, which uses controlled freezing, can also be used for certain lesions, although there is a risk of pigment change or scarring, particularly in deeper plaques. Laser treatments can precisely target xanthelasma and may be a good option for some patients, depending on skin type and lesion characteristics.
For many patients, especially when xanthelasma involves the upper eyelid, surgical excision is often the most direct and definitive approach. Excision physically removes the plaque and can provide immediate improvement, and careful closure is used to support a good cosmetic result. As with any treatment, recurrence can occur over time, so treatment planning also includes discussion of expectations and maintenance.
