Hello, my name is Dr. Thomas Knackstedt, and I’m a board-certified cosmetic dermatologist and dermatologic surgeon at Peak Skin Center. In this video, I want to talk a little bit about cosmetic mole removal, and the difference between a shave removal and an excision.
First, the most important step is making sure the mole is appropriate for cosmetic removal. That starts with a skin exam, and in many cases the removed tissue is sent to pathology to confirm the diagnosis.
A shave removal removes the raised portion of the mole at the level of the skin surface, usually after numbing the area with local anesthetic. It is typically quick, does not require stitches, and the site heals like a superficial abrasion. Recovery usually involves a small open wound that forms a light scab, then pink new skin, and gradual fading over several weeks. Shave removal often works very well for raised moles in areas that heal smoothly, especially concave areas such as parts of the face where the skin naturally curves inward and blends well as it heals. The tradeoff is that shave removal can have a higher chance of recurrence, because deeper mole cells can remain beneath the surface and sometimes regrow.
An excision removes the mole with a margin of skin around and beneath it, and then the site is closed with stitches. Excision is more involved, but it tends to provide a more complete removal and a lower recurrence risk. Recovery includes a sutured incision, activity precautions for a short period, and a linear scar that typically improves over several months. Excision is often favored on convex areas, like the shoulders, chest, upper back, or certain parts of the nose or chin, because shave removals on outward-curving surfaces can heal with more noticeable contour change, while a well-planned excision can create a cleaner, more predictable result.
Both approaches can be excellent when selected thoughtfully, and the best option depends on mole type, location, skin tension lines, and the cosmetic goal.
